Ten members of the same Palestinian family, including four children, were among those killed when an Israeli missile struck the al-Dallu’s home in Gaza City on Sunday.
Israel says its air raids are targeting members and buildings of Hamas, a group that governs Gaza, but most of those affected by the attacks have been civilians.
Move comes after widower protested at inclusion of Galway hospital employees
The announcement was made in Ireland’s Dail parliament just hours after Mrs Halappanavar’s husband Praveen said he would not co-operate with the probe because of three Galway doctors.
He claimed it could not be independent.
The widower’s solicitor Gerard O’Donnell announced he had no faith in the state’s Health Service Executive (HSE), which appointed the team to review the case.
